The programme seeks to support elderly patients beyond hospital treatment by focusing on continuity of care, emotional well-being, caregiver support and healthy ageing.
Pranaam has been designed to simplify healthcare for elderly patients who often face challenges in navigating hospital services, coordinating investigations, attending follow-up appointments and managing chronic illnesses, particularly those living alone or depending on caregivers.
