<p>A review into the Moa Point point wastewater failure in Wellington has blamed it on longstanding weaknesses in governance, accountablity, and risk management - rather than a single decision.</p> <p>The Moa Point plant failed during heavy rain in February this year, when an <a href="https://www.1news.co.nz/2026/03/20/trapped-air-flaw-possible-cause-of-moa-point-wastewater-failure/" target="_blank">outlet pipe became blocked </a>and wastewater then flooded and wrecked much of its equipment.</p> <p>It meant millions of litres of untreated wastewater was discharged on the coast, with <a href="https://www.1news.co.nz/2026/02/25/public-can-return-to-most-of-wellingtons-southern-beaches/" target="_blank">beaches closed</a> to the public for weeks afterward.</p> <p>A technical review of the failure said the the outlet pipe was blocked by an air pocket during high outflow.
