The crash occurred at about 23:30 on Sunday.
Due to the severity of the collision, authorities have closed the N1 in both directions.
Earlier on Monday morning, the provincial Department of Mobility said 18 people had died, and three had been injured.
Authorities have since confirmed that the death toll has risen to at least 21.
The department’s spokesperson, Muneera Allie, said emergency and traffic teams were still on the scene, while the highway remained closed.
Motorists are urged to avoid the area where possible, expect significant delays, and follow the instructions of traffic officials and emergency personnel on site.
