ACC Issues Guidance on Managing Resistant Hypertension

The American College of Cardiology has released a new expert consensus pathway for managing resistant hypertension in adults. The guidance provides clinicians with a framework for accurate diagnosis, standardized monitoring, and the integration of both pharmacological and device-based therapies.
Why it matters
Standardized clinical pathways improve patient outcomes for complex conditions like resistant hypertension by reducing diagnostic errors and treatment variability.
The American College of Cardiology (ACC) has developed a 2026 Expert Consensus Decision Pathway (ECDP) on optimizing blood pressure (BP) care in patients with resistant hypertension, as published in the Journal of the American College of Cardiology .
The ECDP specifically focuses on optimizing BP control in adult patients with resistant hypertension and provides practical guidance for clinicians. The statement addresses best practices for standardization of BP monitoring and measurement in different settings; diagnosing resistant hypertension, including differentiating it from other difficult-to-control BP types, such as pseudo-resistance; using pharmacologic, nonpharmacologic, and device therapies to manage patients with resistant hypertension; strategies to overcome social and societal barriers affecting BP management; and using care teams and digital solutions to manage complex patients with comorbidities and dynamic care as needed.
